For your convenience, you can download your patient forms using the links below to complete and bring to your first visit with us. If you are unable to download these forms, please be prepared to compete all insurance and health information forms at your first office visit which will allow us to begin your dental treatment. We will also ask you to fill out a few forms that will get you acquainted with our office.
If you would like us to have copies of your previous dental treatment, please contact your previous dental provider to request your dental records be transferred to our office.
Your initial dental hygiene exam will last approximately one hour. Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth on this visit. Our goal is to provide you with the best possible dental care while discussing with you your goals for a healthy smile.
